I will ask you to fill in Client Information Form that provides me with some personal information, as well as asking you to sign a Treatment Consent form, which is required as part of the contract in which you agree for me to treat you.  I am also legally obliged to obtain certain information in order to comply with regulations and ensure I am covered by my insurance company.  The information you provide is for my records only and will be treated as confidential, and you have the right to access any information I hold about you.

I do not currently have disabled access to the cabin and access is currently via a step, from the path up to the plinth the cabin stands on, in my garden.
